Calvary Student Ministry will use a NEW "Informed Consent, Liability, and Medical Release" form this year.

This form must be completed annually and notarized for each student. This form is used whenever our youth leave the premises for camp, day trips, and other outings.

Please read and complete one original without your signature, then sign in the presence of a notary. We do have a notary on staff if you should need that assistance.
